If you want additional feedback besides the Certificate of Course Completion, you can ask your instructor to write a 100- to 200-word description of the course with any additional information you wish to have included.
For example, you may wish your instructor to include additional data about your work such as your lesson completion rate, your accuracy rate and the outcome of the Language Achievement Test. For a Writing-Intensive Course, your may wish your instructor to describe the scope of your writing projects and your process of revision.
To assure that this document is accurate and timely, please request your evaluation no later than three months after completing your course. There is a fee of $45.00 for the supplementary Instructor Evaluation.
